Parental Consent & Volunteer Agreement I certify that I am over the age of 18 and understand that all information submitted on this form is considered a public record under Chapter 119 of the Florida Statutes. By completing this form, I give consent for my child to volunteer at the Town of Miami Lakes Cars for Heroes Event on Saturday, October 11th. I acknowledge the following: My child is a high school student and meets the eligibility requirements to volunteer. My child is committed to attending the event and will arrive at the designated Volunteer Check-In area by 4:30 PM. As a volunteer, my child is expected to behave respectfully and responsibly, follow the directions of event coordinators, and complete all assigned tasks to the best of their ability.
