I certify that I am over the age of 18, and I understand that by submitting this form, the information provided is considered a public record, pursuant to Chapter 119 of the Florida State Statutes. By submitting this form, I acknowledge and agree to the following terms and conditions for my child to volunteer at the Town of Miami Lakes Hispanic Heritage Month Celebration. I understand that by submitting this form, I am committing my child to volunteer at the event on Saturday, October 4th. My child agrees to arrive at the Roberto Alonso Community Center (16500 NW 87th Avenue, Miami Lakes) no later than 5:00 PM on the day of the event. I confirm that my child is a high school student and meets the eligibility requirements for volunteering. My child understands that as a volunteer, they are expected to behave in a respectful and responsible manner, follow all instructions from event coordinators, and complete their assigned tasks to the best of their ability.
