WHAT MUST I BRING

To apply, renew, or replace your license or ID, bring:

ONE PRIMARY ID

For Miami Lakes Residents

• U.S. Citizens: Birth certificate, valid U.S. passport, or naturalization certificate
• Legal Residents: Green card (Form I-551) or USCIS approval notices
• Temporary Status: Work authorization (Form I-766), valid I-94, or travel documents with
• passport

ONE PROOF OF SOCIAL SECURITY

• Social Security card, W-2, pay stub, or 1099 (no handwritten documents)

TWO PROOFS OF ADDRESS (dated within 60 days)

• Utility bill, lease/mortgage, vehicle registration, bank statement, employer or government correspondence
