I certify that I am over the age of 18, and I understand that by submitting this form, the information provided is considered a public record, pursuant to Chapter 119 of the Florida State Statutes. By submitting this form, I acknowledge and agree to the following terms and conditions for my child to volunteer at the Town of Miami Lakes Hispanic Heritage Month Celebration. I understand that by submitting this form, I am committing my child to volunteer at the event on Saturday, October 10th. My child agrees to arrive at the Main Street (15301 NW 67th Ave, Miami Lakes) no later than 2:00 PM on the day of the event. I confirm that my child is a high school student and meets the eligibility requirements for volunteering. My child understands that as a volunteer, they are expected to behave in a respectful and responsible manner, follow all instructions from event coordinators, and complete their assigned tasks to the best of their ability.
